Your Appointment

Listed below is useful information about our practice and what you'll need to know for your appointment. To access the highlighted information, you must have the Adobe Acrobat Reader^TM. • Patient/Medical History Forms

• Your insurance card
• A list of current prescription or over-the-counter medications you are taking, including dose and frequency
• Pertinent information about your medical and surgical history; bring any pertinent x-rays or records you may have
• Completed patient and medical history forms

Payment/Insurances Accepted

• Medicare
• Most HMO and PPO plans

We are happy to process all insurance forms for you as long as you have provided us with the appropriate insurance information. We request payment at the time of your appointment for services that are not covered by insurance and any co-payment that may be required by your insurance plan. We expect payment promptly if you receive a bill from our office for balances that may be due. Please feel free to discuss any questions you may have about billing with our office staff.

Western Main Line Urology is a provider for most major insurance plans. However, it is in your best interest to check your insurance providers list of participating physicians to be sure your plan allows you to be seen at our office. Also, please review with your insurance customer service representative (or insurance provider) the rules and limitations on coverage and reimbursement.
